Your NDIS plan will be focused on you. It will include information about your own aspirations and needs, as well as information about the supports you will be funded for and the amount of funds you will receive.

• One of our support coordinators will have a consultation with you in your home and get an idea of which services we can assist you with.
• We will develop a service agreement together, outlining the supports you need.
• Receive on-going support services from us tailored to your needs.
